• The F.A. Program is a spiritual program. The information presented is an opinion and does not constitute any mental health or medical advice. The F.A. Program is based upon spiritual principals and is not intended to diagnose, treat, cure, or prevent any condition or disease. Please seek advice from your healthcare professional for the personal health concerns of the minor prior to taking advice from the F.A. Program and its members and volunteers.
• A Sponsor in the F.A. Program can be a volunteer from an outside 12-Step program to sponsor the minors. The Sponsors volunteer to work with the minors.
• Due to the size of the F.A. Program, Foster Anonymous, Inc. does not background check or interview the Sponsors. The F.A. Program provides the contact information for the Legal Guardian, Social Workers, and Case Workers to interview the Sponsor. The Legal Guardian is responsible for obtaining a Level 2 Background Check of the Sponsor for the safety of the minor at their own expense.
• Legal Guardian shall hold Foster Anonymous, Inc. or any of its affiliates harmless for any action of volunteers, sponsors, members, or participants involved in the F.A. Program.
• By electronically signing this consent form and waiver (via e-document signature), I consent to the minor’s participation in the F.A. Program including live or on-line meetings, sponsor meetings, and other activities.
• By signing this consent form, I confirm I am legally responsible and competent to contract on behalf of the minor.
